Our office is open for appointments starting Monday, January 17, 2022, however the IRS will not begin accepting tax returns until Monday, January 24, 2022. If you have received all of your tax documents or any letters from the IRS that may pertain to you, you can get ahead of the crowd and have your tax return waiting in queue for the moment the IRS and State of Oklahoma begins accepting tax returns!

CHECKLIST

General Information

  • Driver’s License/State ID

  • Social Security Card (Not required but recommended)

  • W-2 Forms

  • 1099 Forms (Retirement, SSI, Unemployment, Contract Labor/Self Employment)

Health Care

  • Any letter or documents from Marketplace

Education

  • 1098-T Form

  • Breakdown of College Expenses

  • Daycare Tax ID and Amount Paid (We will no longer be able to look up last year's information so you must provide this.)

Dependent Documentation 

(At least ONE from the list showing the dependent's address)

  • Current Year School Records

  • Social Service Records (DHS)

  • Medical Records

  • Landlord  Statement

  • Childcare Records

  • Employer Statements

  • Placement Letter (for non-biological children)

Miscellaneous

  • Mortgage Interest Statement

  • Direct Deposit Information

  • IRS Letters 6419 & 6475 (Child Tax Credit and Stimulus Payment amounts received). Or, log on to IRS.gov and select “View Your Child Tax Credit Payments” and document the amounts shown.
